Domestic Violence
- RADAR, a domestic violence intervention tool, includes steps for physicians to follow at every visit and contact information for national and Pennsylvania-specific resources.
Diabetes
- The Diabetes Mellitus Flow Sheet, provided by the Pennsylvania Medical Society, can be used by physician offices to track annual visit interventions including a complete history and physical, foot exam, dilated eye exam, microalbuminuria, immunizations, and adherence to aspects of self-care.
