 HHQI Reports Quick Check

The QIO LANE responsibilities for the HHQI Campaign differ from those of the state associations in relation to the HHQI Campaign Reports. As the experts in interpreting and giving meaning to outcome data, the QIOs are responsible for supporting and providing the monthly HHQI Campaign Reports to the home health agency campaign participants.

The HHQI Reports “Quick Check” is a 13-question self-assessment of the monthly campaign report. It is now available on the HHQI Web site under For Home Health Agencies. You may want to use this when reviewing HHQI Reports with your agencies.

QIO Home Health Educational Sessions for Medicare Advantage
A collaborative working relationship with payers may foster QIO-sponsored initiatives, improve patient outcomes and be valued by your home health care providers. The home health QIO representatives from California, New York, Ohio, Texas and Pennsylvania worked together to develop resources to support QIO/payer relationships through education. These materials are for optional use should you identify a need for education to managed care payers within your state.

The following resources are now available:

  • Introductory Letter Template - Describes the HHQI National Campaign and offers educational opportunities for Medicare Advantage staff (modifiable Word document)
  • Announcement Flyer - Describes target audience and objectives for each session
  • HEDIS/Home Health QI Crosswalk - Demonstrates correlation between health care payer and home health initiatives and resources
  • Medicare Home Health Services: The Basics - PowerPoint presentation; Can easily be modified to use with hospital discharge planners
  • Home Health & Medicare Advantage: Working Together to Improve Patient Outcomes - PowerPoint presentation
The two PowerPoint presentations are on blank templates for you to personalize for your QIO. You may choose to offer as an onsite presentation, teleconference or live/pre-recorded WebEx. Each session qualifies for 1.0 CNE for registered nurses or 1.0 contact hours for certified case managers (beginning November 1, 2007 - July 31, 2008). These resources can be found on MedQIC under Care Transitions.
